Camper Health Form

Before you begin this form, please have the following available, scanned, and ready to upload as an attachment.

• Your Camper’s Insurance Card

• A Copy of your Camper’s latest medical physical clearing them for camp activities that was done in the last 18 months before the start of camp.

• Your Camper’s Immunization Records

  • If your camper has a medical exemption to a immunization, please have that ready to upload, too.

  • If your camper has a religious exemption to immunization, you must have a signed letter from the parent/guardian stating in writing that a vaccine or vaccines conflicts with their sincerely held religious beliefs.

One Camper Health Form must be completed for each camper. Health form must be completed at least 2 weeks before camp start.
